Dental Insurance	Full time and Part time regular employees, immediately	ABC and Employee	Group insurance for employee and/or family. Employees contribute portion of premiums.
Vision Insurance	Full time and Part time regular employees, immediately	Employee	Group insurance plan available for employee and/or family.
Health Care Reimbursement Account	Full time and Part time regular employees, immediately	Employee	Options to pay certain health, dental and/or vision expenses with “before” tax dollars and reduce your federal, state and Social Security taxes.
Dependent Day Care Reimbursement Account	Full time and Part time regular employees, immediately	Employee	Options to pay dependent day care expenses with “before” tax dollars and reduce your federal, state and Social Security taxes in the process.
401(k) Retirement Program	All regular and temporary employees	Employee	A tax deferred 401(k) plan. You may elect to contribute up to the IRS limits to the plan on a pretax basis.

Basic Life Insurance	Full time regular employees, immediately	ABC	One times the employee’s base annual salary.
Accidental Death and Dismemberment	Full time regular employees, immediately	ABC	One times the employee’s base annual salary in the event of the employee’s accidental death.
Supplemental Life Insurance	Full time regular employees, immediately	Employee	Options from $10,000 to $500,000 for employee coverage. Spousal coverage at ½ of employee coverage. Child(ren) coverage at $10,000.
Long Term Disability	Full time regular employees, immediately	ABC	Monthly benefit for employees who become totally disabled from work for more than 60 days by accidental bodily injury, sickness or pregnancy up to 60% of annual salary.
Vacation Time	Full time regular employees, immediately	ABC	Accrual starts on the 1st day of employment and you accrue 10 days in the first year. Staff accrues 15 days after 5 years, 20 days after 10 years and 25 days after 20 years of service. Maximum accrual is equal to your annual accrual plus 5 days.
Sick Leave	Full time regular employees, immediately	ABC	Accrual starts 1st day of employment and you accrue 1 day per month. Maximum accrual 42 days.
Holidays	Full time and part time regular employees immediately	ABC	ABC observes 9 paid holidays per year.
Personal Day	Full time regular employees, immediately	ABC	ABC provides you with one day to take care of personal business or to add to your vacation on your anniversary each year.
Annual Blood Screening	All regular employees	ABC	Provides information about potential health risks in addition to continuing medical surveillance for your personal safety.
Bereavement Leave	Full time regular employees, immediately	ABC	Provides for 3 regularly scheduled work days missed due to the death of an immediate family member.
Bonus/Incentive Program	All regular employees	ABC	A percentage of after tax points is divided equally amount participants in the general staff program and paid in cash following publication of the company financial results. If determined by Board.

Direct Deposit	All employees, immediately	Employee	Payroll wages are directly deposited to a designated bank or credit union.

Lab Coats and Safety Glasses	All Employees	ABC	ABC provides lab coats and safety glasses to all staff. Free laundry service available for lab coats.
Parking	All Employees, immediately	ABC	Free parking available.
Tuition Reimbursement	Full time regular employees after 1 year of service	ABC	Reimbursement must be preapproved by ABC. Expenses reimbursed at 50% for individual courses directly related to work and required textbooks.
